Through lifestyle intervention and education, many people can better manage their illness and make changes in their daily habits that can delay or even prevent disease.

At WellAware, the client enters into a cooperative relationship with a caring registered nurse. Together, they explore the client's personal health risks and alternatives to enhance health. They also work collaboratively to set goals to improve self-care management and promote wellness.

Nurses meet electronically, through telephone conversations, or at the workplace, to reassess each associate’s needs, monitor progress, and re-evaluate goals to keep associates motivated to make healthy changes. The nurse provides the client and family with education about medications, early signs and symptoms of complications, disease management, and wellness promotion. The WellAware nurse helps the associate identify barriers and set specific goals.
